They’re non-diagnostic and designed to give you helpful insights, so you can better understand how you’re feeling and build positive habits over time.
How to take an assessment
Step one – Log in to your Perkbox account.
Step two – Select Wellbeing from the main menu.
Step three – Select Assessments.
Step four – Choose the assessment you’d like to take.
Step five – Read the short introduction explaining what the assessment covers.
Step six – Select Start assessment.
Step seven – Answer each statement by choosing from options such as:
Strongly agree
Agree
Disagree
Strongly disagree
Step eight – Once you’ve completed all questions, review your results.
Understanding your results
At the end of the assessment, you’ll receive:
A wellbeing score
A short explanation of what your score means
Personalised tips and next steps
Your results are designed to help you build healthy habits over time. You can retake the assessment whenever you want to check your progress.

Yes. The Wellbeing Index assessment uses the non-diagnostic WHO-5 scale, developed by the World Health Organization, to measure wellbeing rather than mental illness.
The social wellbeing assessment is built using the Lubben Social Network Scale (LSNS-6) — a validated clinical tool designed to identify social isolation by measuring the size, closeness, and frequency of contact within your social network
All data is anonymised and handled in line with GDPR.
